The possible link between hypertension and chronic kidney disease has been preoccupying the medical world for over a century. The cause-effect relationship between malignant hypertension and renal disease is clear and agreed upon, but when hypertension is at milder levels, this relationship becomes blurred. Does indeed non-malignant hypertension also cause kidney disease that can lead to end-stage kidney failure? In this review, we provide evidence based primarily on epidemiologic data, from which it can be concluded that the higher the blood pressure is, starting from normal blood pressure and above, the higher is the risk of developing kidney disease and loss of renal function. Does treatment of hypertension prevent kidney injury secondary to hypertension? The evidence points to the issue that hypertension slows down the rate of decline in glomerular filtration rate, but only in the presence of significant urinary protein excretion, whereas in the absence of proteinuria, lowering blood pressure has no effe of proteinuria, lowering blood pressure has no effect on the development of hypertensive renal disease.

This educational endeavor promotes the development of new instructive platforms for trainees as well as launching evaluation strategies for resident professional development. The cost of surgical education was never conveyed to the attention of medical policy makers. The financing for educational purposes during residency was perpetually inconsistent and fluctuant. Changing residency teaching techniques and domains pose financial burdens. Financing for residency education should be the interest of the various stakeholders e.g. Ministry of Health, medical centers and medical insurance agencies. Dedicated philanthropy should be directed to support the transfer of surgical residencies to competence based surgical tutoring but never as a singular source.

Annually in the US, about 2 million patients who are animal-related casualties go to the Emergency Medicine Department (ER), which accounts for 0.5-1.5% of the ER visits; 25% of these casualties are children. Similar numbers are also seen in Europe and the UK. In a single-center study in Israel, it was found that the percentage of ER visits for animal injuries was about 2.1%. The COVID-19 pandemic is a demonstration of an unforeseen event that disrupts normal life routine and greatly affects medical training. With its outbreak, clinical studies in medical schools were discontinued throughout the country, and the pre-clinical studies channeled into online learning, like all other university teaching activities. Since similar situations occurred in most western countries, a methodological approach of online teaching is required – principles, goals and implementation, with reference to the tension between the duty of care versus the duty of maintaining the clinician’s and student’s health. In this paper we will present the dilemmas faced by the various medical education institutions in the world, along with the ways of coping and the experience gained from similar situations – including in Israel. We will discuss the techno-pedagogical principles that are at the heart of online teaching, demonstrate the experience gained in Israel and around the world, and offer a framework for comined from similar situations – including in Israel.
