Patients received either three sessions of radial-Extra-Corporeal Shockwave Therapy (one session per week for 3 weeks), or two ultrasound-guided Autologous Blood Injections (2 injections performed 2-4 weeks apart).     Fractures are increasing worldwide and with an aging population, are frequent in the elderly. The healing of fractures progresses through various phases including the inflammatory stage. Aging is associated with slower healing and the use of non steroidal anti-inflammatory drugs (NSAIDs) may interrupt bone healing processes. We designed a study to compare the effect of diclofenac and celecoxib on fracture callus histomorphometry in a rat model of different age groups.

    Using 5 and 15 month old rats, fractures were induced on the left tibia and the animals allocated to receive one of the drugs. Animals were sacrificed at day 21 and 42 and the fracture callus harvested for processing and histological evaluation. Tissue proportions and histological grades were determined and compared across the groups.

    Across all groups, the histological grade increased with time and animals in the young diclofenac group had the highest grade at day 42 (p=0.004). The proportion of bone increased in all groups and was highest in the young diclofenac group at day 21 and day 42 (p=0.003). Post hoc analysis showed that the young celecoxib and old celecoxib groups had the least proportion of bone (p=0.032 and p=0.003). The proportion of cartilage reduced in all groups at both time points.

The review initially introduces four endotypes (i.e., a more collapsible airway, upper airway muscle responsiveness, arousal threshold, and loop gain) that may have a role in the initiation of obstructive sleep apnea. Thereafter, sex differences in the prevalence of sleep apnea are considered along with differences in the prevalence that exist between younger and older women. Following this discussion, we consider how each endotype might contribute to the increase in prevalence of sleep apnea in aging women. Lastly, we address how modifications in one form of respiratory plasticity, long-term facilitation, that might serve to mitigate apneic events in younger women may be modified in aging women with obstructive sleep apnea. Overall, the published literature indicates that the prevalence of sleep apnea is increased in aging women. This increase is linked primarily to a more collapsible airway and possibly to reduced responsiveness of upper airway muscle activity. Catalase (CAT) delivery can be effective in inhibiting reactive oxygen species (ROS)-mediated prolongation of the wound healing process. In this study, to improve CAT stability for effective application, gelatin(Gel)-alginate (Alg) biocompatible hydrogel (Gel-Alg), as immobilization support, was prepared using calcium chloride as an ionic cross-linker. High entrapment efficiency of 92% was obtained with 2% Gel and 1.5% Alg. Hydrogel immobilized CAT (CAT-Gel-Alg) showed a wide range of pH from 4 to 9 and temperature stability between 20 to 60 °C, compared to free CAT. CAT-Gel-Alg kinetic parameters revealed an increased K m (24.15 mM) and a decreased V max (1.39 µmol H2O2/mg protein min) × 104. CAT-Gel-Alg retained 52% of its original activity after 20 consecutive catalytic runs and displayed improved thermal stability with a higher t 1/2 value (half-life of 100.43 vs. 46 min). In addition, 85% of the initial activity was maintained after 8 weeks’ storage at 4 °C. At 24 h after thermal injury, a statistically significant difference in lesion sizes between the treated group and the control group was reported. Finally, our findings suggest that the superior CAT-Gel-Alg stability and reusability are resonant features for efficient biomedical applications, and ROS scavenging by CAT in the post-burn phase offers protection for local treatment of burned tissues with encouraging wound healing kinetics.This study cross-compares ICON/MIGHTI and Thermosphere, Ionosphere, Mesosphere Energetics & Dynamics (TIMED)/TIMED Doppler Interferometer (TIDI) MLT region neutral winds from middle Northern Hemisphere to low Southern Hemisphere latitudes. We utilized MIGHTI level-2.2 (v4) and TIDI level-3 (v11) neutral winds from January 2020 to November 2020 and found their conjunctions using a space-time window of LST ± 15 min, latitude ± 4°, and longitude ± 4° around each TIDI wind measurement. Due to the nature of their orbital geometry, frequent conjunctions occurred between MIGHTI and TIDI. These conjunctions are spread in longitudes and they occur at approximately fixed LSTs and latitudes, which allows us to compare their observed diurnal variability. MIGHTI and TIDI wind observations agree well (except on the TIDI coldside during forward flight) and show similar large amplitude longitudinal variations that can reach more than 100 m/s. MIGHTI and TIDI zonal and meridional winds show moderate correlations of 0.60 and 0.55, respectively. The slopes of regression fits for zonal and meridional winds are 0.92 and 0.91, respectively. The root mean square differences in zonal and meridional winds are 56 and 66 m/s, respectively. We found that TIDI coldside measurements in forward flight show a systematic bias and this behavior is repetitive as the instrument pointing direction is changed by the periodic TIMED yaw maneuver. The nature of this systematic bias suggests that the TIDI zero-wind references (at least for the coldside telescopes) need revision. This investigation can provide guidance toward improving the TIDI data analysis. In addition, the results of this study act as a validation of MIGHTI MLT winds.

    Management of comminuted facial fractures with maxillary-mandibular arch interruption is difficult, resulting in inadequate bone reduction and malocclusion. Traditionally, a good quality dental splint is helpful, but difficult to obtain in acute trauma. We apply a computer-assisted design and three-dimensional printing technology to improve splint fabrication and utilization, thus facilitating restoration of dental occlusion and facial fracture.

    We retrospectively reviewed patients who suffered from facial fractures with interruption of the maxillary-mandibular arches. We developed the “computer-assisted reverse planning and three-dimensional printing model surgery” algorithm and applied it in selected patients. An occlusal splint was created as a surgical guide to enhance the maxilla-mandibular unit repair by taking care of the bone reduction and occlusion. All included patients were followed up to assess the functional outcome and patients suitable for this method.

    Static cold storage is the gold standard of preservation in vascularized composite allotransplantation and allows a preservation time of 4-6 hours. Machine preservation is a promising technique for prolonged preservation; however, studies on extended preservation that compare different preservatives are scarce. This study aims to assess the feasibility of 24-hour acellular perfusion and compares different preservation solutions in a porcine myocutaneous flap replantation model.

    Six harvested bilateral myocutaneous flaps of three Dutch Landrace pigs were perfused hypothermically for 24 hours with University of Wisconsin machine perfusion solution (UW-MPS; n = 2) or histidine-tryptophan-ketoglutarate solution (HTK; n = 2) or preserved on ice for 4 hours (n = 2) before orthotopic replantation. Animals were observed for 7 days after replantation. Skeletal muscle injury was assessed by biochemical markers during perfusion, and muscle biopsies were analyzed for ischemia reperfusion injury directly after preservation and at 1, 3, and 7 days after replantation.

    Markers of muscle damage varied during perfusion, but decreased overall in both perfusion groups. Flap weight increased 60% and 97% in the HTK-perfused flaps, compared with -6% and -7% in the UW-MPS-perfused flaps after 24 hours. Histopathologic evaluation demonstrated decreased muscle damage in flaps perfused with HTK compared with the UW-MPS-perfused flaps at 1 week after replantation.

    It is well established in the literature that the satisfaction of trainees correlates with higher productivity and efficiency during training years. The satisfaction rate of Saudi Board of Plastic Surgery trainees has not been investigated since its establishment in 2001. Therefore, we aimed to measure the satisfaction rate of local trainees and detail the predictors of satisfaction.

    This study was conducted on September 9-13, 2021. The study is a cross-sectional, nation-wide study utilizing a self-structured questionnaire targeting all plastic surgery residents in Saudi Arabia. IRB approval and approval from the governing body of residency programs in Saudi Arabia were obtained.
